Fertility and assisted reproduction treatments
Intracytoplasmic Sperm Injection (ICSI)
This technique was developed for those cases in which sperm concentrations were too low for classical in vitro fertilization or for those in which the results of the fertilization of the oocyte weren’t good. It basically consists of injecting a single sperm directly into a mature oocyte (rather than allowing them to try and fuse naturally). In this way only one live sperm is needed per oocyte so that it allows working with semen samples with very low sperm concentration or using other techniques to recover sperm directly from the testicle such as PESA (percutaneous epididymal sperm aspiration) or TESA (testicular sperm aspiration).
Embryo Cryopreservation
This consists in freezing good quality embryos that remain after embryo transfer, with previous couples’ authorization. Thus, if the couple have frozen embryos, they doesn’t need to repeat completly the IVF treatment. Those embryos are thawed at the time they will be transferred to the woman. The transfer is done in the same way as if there were fresh embryos as long as they keep good quality after the thaw.
Sperm Cryopreservation
Sperm cryopreservation is available for men who wish to bank sperm for possible future use. During cryopreservation, the sperm is frozen and then stored in liquid nitrogen at very low temperatures. Men with certain conditions in which sperm have to be extracted through surgical means, or those who undergoes a vasectomy or a chemotherapy treatment, may consider freezing sperm for future use. Frozen sperm remain viable indefinitely and can be thawed for use in future fertility treatments.
Surgical Sperm Retrieval
Surgical sperm retrieval may be a treatment option for men with an obstruction preventing sperm release or for those with non-obstructive azoospermia (complete absence of sperm). In the first case it is possible to collect sperm directly from the epididymis by percutaneous epididymal sperm aspiration (PESA). In the second one, sperm can be collected directly from testes by performing a testicular biopsy (TESA). This microscopic core of tissue is then prepared by biologists to release the developing sperm.
Assisted Hatching
This is a complementary technique for IVF treatments. It is a treatment of the zona pellucida to try to improve implantation. For implantation to occur, the embryo must escape from its outer coat (the zona pellucida) – this is known as 'Hatching'. Once the embryo has hatched through the zona pellucida it can make physical contact with the lining of the womb and implantation begins. Failure of implantation may result from an inability of the embryo to hatch and in some women this may be why they don’t get pregnant, even with IVF.
Blastocyst Culture
Embryos are cultured for 5 days before being transferred into the uterus. This allows the embryologists to select more advanced embryos with better potential for implantation at the time of the transfer. It is only performed in special cases.
Preimplantation Genetic Diagnosis (PGD)
PGD is used for diagnosis of a genetic disease in early embryos prior to impantation in the uterus. This technology can also be used in the field of assisted reproduction for diagnosis of genetic abnormalities
